Ok a reverse setup simply means you are creating in the opposite direction, for example a leg,, you create the hip bone then draw out the rest till you reach the toe. Now reverse that, start off with the toe and go up. Stop at the hip. Snap the leg to the hip by merging the vertices in modeller. The hirec... will start off from the toe so there should not be any problems with the hip. As for the IK, from the hip to the ball should be fine.
